A Tennessee Department of Health Certificate of Immunizations (see example) is required for all Kindergarteners, 7th graders, and new students registering for Endeavor Microschool.

 

Following State of Tennessee law, medical and religious exemptions are allowed.

 

Medical exemptions must be supported by documentation from a licensed physician.

Religious exemptions must be accompanied by a signed, written statement that vaccinations conflict with the religious beliefs or tenets of the student and/or parent/guardian.
